= Report IMMEDIATELY by telephone.
Dial (530) 225-5591 on weekdays between 8 a.m. and 5 p.m., and (530) 395-0132 after hours, weekends, and holidays.
= Report within 1 WORKING DAY of identification by Phone, Fax (530) 225-5074, or mail to Shasta County Public Health, 2650 Breslauer Way, Redding, CA 96001.
All other Diseases/Conditions should be reported by Phone, Fax or mail within 7 calendar days of identification using the appropriate Confidential Morbidity Report form. These forms can be filled out online, then faxed to our confidential fax number.

Use this form for reporting all conditions except Tuberculosis and conditions reportable to DMV.

Use this form for reporting lapses of consciousness or control, Alzheimer's disease or other conditions which may impair the ability to operate a motor vehicle safely (pursuant to Health and Safety Code Section 103900).
