VIDEO: Lori's Whole Person Care Success Story

Because she was battling with health issues and homelessness, Lori, a Shasta County resident, was able to get a referral to HHSA's Whole Person Care program.  

A small team - a housing case manager, medical case manager and a registered nurse - helped Lori deal with her complex health needs, which included finding a safe and affordable place to live.
