Births

Shasta County Public Health keeps birth certificates on file for only three years, the current year and previous two years. Copies may be obtained at our office at 2650 Breslauer Way, or by calling (530) 225-5063. For all birth certificate requests please complete the Application for Certified Copy of Birth Record. The cost is $17 per certificate.

Home Births

All home births must be registered within the first year. For information on amendments, acknowledgement of paternity, delayed birth certificates, or for appointments, please call (530) 225-5063.

Deaths

The Public Health Department keeps death certificates on file for only three years; the current year and previous two years. Copies my be obtained at our office at 2650 Breslauer Way, or by calling (530) 225-5063. For all others, contact the Shasta County Recorder's Office at (530) 225-5678. All death record requests must have a completed Application for Certified Copy of Death Record. The cost is $12 per certificate.

How to Get a Burial Permit

Shasta County Public Health issues burial permits to the general public or through cemeteries and funeral directors. The cost is $11.
