Multi-Disciplinary Team

The team meets weekly and reviews all cases coming into the system through the intake process and reviews cases already in the system at decision-making times.

The team monitors the decisions being made by social workers to make sure that the decisions made are within the law and are in the best interest of the child and family.

The team is made up of representatives from Probation, Children and Family Services, County Counsel, Mental Health, Superintendent of Schools, Family Court, Consulting Psychologist, Foster Parent Representative, Public Health, and Substance Abuse.

Intensive Services Team

Children and Family Services utilizes teams of social workers, and community resources staff to provide intensive treatment planning and case supervision to at risk children and families. The goal of these teams is to reduce multiple placements, provide individualized services and increase family/community involvement by:

· Providing detailed child and family assessments

· Designing specialized treatment planning for children, families and community resources

· Locating the most suitable placement based on individual, child and family needs, services and location

· Preventing placement failure by anticipating struggles and supporting the child, family and care providers during periods of difficulty

· Intensive case supervision, from an inter-agency perspective, so that child connections and continuity in care may be maintained

· Responding to family and child strengths while implementing flexible treatment planning.

Wraparound was created by Senate Bill 163, and it authorizes participating counties to use a portion of foster care funds to flexibly and creatively implement individualized services for eligible children. The program is administered by the Shasta County Department of Social Services in collaboration with the Shasta County Departments of Mental Health, Public Health, and Juvenile Probation and the Shasta County Office of Education as well as local community providers.

The Wraparound process is a team with shared goals and is built on family strengths. It is designed by the family and team to meet individual needs and is based in the family’s community with the family’s own support systems. Wraparound includes many services, such as a 24-hour crisis support, in-home family specialist, respite options, parent partners and flexible funding. The program acknowledges the value of each person and helps people engage with each other. It identifies resources to build an effective support plan and provides concrete proof of potential. Wraparound creates a spirit of commitment and shared risk taking and utilizes what works, with both traditional and non-traditional services.

The children in Wraparound include those in, or at risk of, high-level group home placement and:
· Wards of the Court (W&I 602)
· Dependent of the Court (W&I 300)
· Certified eligible for Mental Health services (AB3632)

Wraparound services are voluntary. Both the family and the agency need to agree to participate and the family/caregiver must live in Shasta County. Referrals are accepted from Shasta County Juvenile Probation, Children and Family Services and Shasta County Mental Health (which refers services via an expanded individual education plan team). The Wraparound Team and Placement Prevention & Resources Team meet regularly to review cases for enrollment into the program.
